Document Requirements:

1. Latest original 3 months bank statements & bank certification
2. Latest copy of Barangay Business Permit
3. Latest copy of DTI Permit
4. Photocopy of at least two (2) Government-issued IDs
5. 2" x 2" ID picture
6. Pictures of business and/or inventory
7. Latest Proof of Billing for your business and residence (e.g. electricity, water, cable)
8. Hand-drawn sketch of your business and residence location

• Additional Requirements for Corporations:
  1. SEC Registration with Articles of Incorporation & By Laws
  2. Latest copy of General Information Sheet
  3. Latest copy of Mayor's Permit
  4. Secretary's Certificate with Board Resolution (Authority to obtain loan in his/her personal capacity)
